Eye Screening Consent Form

The health office will begin hearing and vision screenings next week- Tues (9/28), Wed (9/29) and Friday (10/1).  The Winger Lions Club will be assisting.  We cannot screen your student without a completed consent form, you need one per family.  Any questions, please call the school health office.
